Indiana CMCO Case Management: The 5 Organizations and How to Choose

On August 1, 2026, Indiana eliminated every legacy case management provider from its BDS waiver system and replaced them with five designated Case Management Organizations. If you receive services through the Family Supports Waiver or Community Integration and Habilitation waiver, your case management now runs through one of these five CMCOs — if you have not chosen one yet, contact BDS for the current selection instructions.

The Five Authorized CMCOs

Under the state's federally approved 1915(b)(4) selective contracting model, all BDS waiver case management is now restricted to:

1. Aging & In-Home Services of Northeast Indiana, Inc. — historically focused on the Fort Wayne and northeast Indiana region, with roots in aging services that expanded into developmental disability case management.

2. Indiana Professional Management Group (IPMG) — one of the largest case management providers in the state, operating statewide with established relationships across most BDS districts.

3. Inspire Case Management — a newer organization that emerged from the restructuring, focused on person-centered planning and community integration outcomes.

4. Unity of Indiana — a statewide provider with a track record in both FSW and CIH case management.

5. The Columbus Organization — a national organization with Indiana operations, bringing experience from multiple state waiver systems.

What a Case Manager Actually Does

Your CMCO-assigned case manager is responsible for developing and maintaining your Person-Centered Individualized Support Plan (PCISP), coordinating with waiver service providers, conducting required home visits, and filing the documentation BDS needs to keep your waiver services active.

Under the August 2026 amendments, case managers must now complete all case notes within 7 calendar days of an activity or event. This deadline exists because delayed documentation was causing billing errors and service authorization lapses under the old system.

Your case manager also acts as the liaison when your interRAI assessment is due for renewal, when you need a service authorization change, or when a provider issue arises that could interrupt care.

How to Choose

If your previous case management provider was not one of the five CMCOs, you should have received a transition notice from BDS with a deadline to select a new organization. If you missed that notice or did not respond, contact BDS to confirm your current CMCO and next steps.

When evaluating your options, consider:

Geographic coverage. Some CMCOs have stronger staffing in certain BDS districts. Ask each organization how many case managers they have in your county and what their current caseload looks like. High caseloads mean slower response times.

Specialization. If your family member has complex behavioral support needs or dual diagnoses, ask whether the CMCO has case managers with experience in those areas.

Communication style. Some families want regular proactive check-ins; others want a case manager who handles the paperwork and reaches out only when action is needed. Ask about their standard contact frequency and how they handle urgent issues outside business hours.

Transition support. Ask the CMCO whether they will help transfer your existing PCISP data from the outgoing provider or whether you need to bring documentation yourself.

What to Do If You Are Unhappy

You can request a CMCO change by contacting your local BDS District Office. Indiana’s waiver materials state that individuals and families may change their CMCO; ask the office for the current transfer process. Document any service disruptions caused by the transition — these records matter if you need to escalate an issue to BDS leadership or file a formal complaint.
