Colorado Case Management Agency Directory: Find Your CMA by County

If you're trying to access disability waiver services in Colorado and someone tells you to "contact your CCB," that advice might be two years out of date. Colorado completed its Case Management Redesign in mid-2024, splitting the old Community Centered Board system into two separate functions. Your Case Management Agency (CMA) now handles all Medicaid waiver intake, eligibility, and service planning. Your CCB — if it still operates in your area — handles non-Medicaid local programs like the Family Support Services Program and county mill levy funds.

The distinction matters because contacting the wrong agency wastes weeks. The CCB can't start your waiver application. The CMA can.

How to Find Your CMA

The official source is the HCPF CMA Directory, which lists the designated CMA for each Colorado county. Here are the major regional assignments:

For counties not listed here — particularly rural areas on the Western Slope and Eastern Plains — check the HCPF directory directly. Some geographic catchment areas span multiple counties under a single CMA.

What the CMA Does

Your CMA is the single administrative entry point for all 10 HCBS waivers in Colorado. They handle:

• Intake and eligibility determination — developmental disability determination, functional level-of-care assessments
• The PMIP — they require a completed Professional Medical Information Page before scheduling intake
• Support Level assessment — the Interim Support Level Assessment (ISLA) assigns a Support Level that sets the authorized service budget
• Person-Centered Support Plan — the individualized plan of services, reviewed at least annually
• Waiver enrollment — SLS, DD, and other HCBS waivers
• Ongoing case management — service coordination, support-plan follow-up, and reassessments

CMA vs. CCB: What Changed

Under the old system, Community Centered Boards did everything — intake, service planning, and direct service delivery. A single agency decided what services you needed and then provided (and billed for) those services. Federal conflict-of-interest rules flagged this as a problem: the entity authorizing your care shouldn't also profit from delivering it.

The Case Management Redesign separated the two functions:

• CMAs — conflict-free case management only. They assess needs, develop plans, and monitor services. They do not deliver direct services.
• CCBs — some still exist as direct service providers or administrators of non-Medicaid local programs (mill levy funds, Family Support Services). They no longer handle waiver intake or case management.

In some counties, the CMA and CCB are actually the same organization (like Developmental Pathways in Arapahoe County), but operating under separate contracts with a firewall between the case management and service delivery functions.

Starting the Intake Process

1. Identify your CMA using the HCPF directory
2. Call to request an intake packet — ask specifically about HCBS waiver enrollment (SLS is the most common starting point for new adults)
3. Complete the PMIP — have a licensed medical professional review and sign the medical portions
4. Schedule the intake assessment — the CMA will assign a case manager to conduct the evaluation

If your family was working with a CCB before the redesign and received a letter redirecting you to a CMA, your case records should have transferred. If they didn't — which has happened — bring copies of any prior evaluations, service plans, and eligibility determinations to the CMA intake.
