The PG-500 Petition Packet

Every adult guardianship case in Alaska starts with the PG-500 packet, filed with the Probate Division of the Alaska Superior Court. The packet bundles the core documents the court needs to open a case:

• PG-500 Cover Sheet — identifies the case type and parties
• PG-100 (Petition for Appointment of a Guardian for an Adult) — the substantive petition detailing the respondent's cognitive deficits, the specific areas where they need support, and evidence that less-restrictive alternatives have been attempted
• PG-115 (Notice of Guardianship Hearing) — the formal hearing notification served on the respondent and interested parties
• PG-117 (Certificate of Service by Certified Mail) — proof that all statutorily required people have been notified

The PG-100 petition must be fully notarized before filing. The court clerk will reject it without a notary seal — no exceptions.

Post-Appointment Forms

Once a guardian is appointed, the court imposes strict reporting deadlines:

PG-120 (Affirmation of Mandatory Education) — due within 30 days after the court distributes the guardianship order. All newly appointed family guardians must complete at least one hour of training on their fiduciary duties. The Alaska Court System offers this online or on physical flash drives.

PG-401 (Guardianship Plan) — due within 30 days after the court distributes the guardianship order. A written plan covering how the guardian will care for the ward, encourage participation in decisions, and protect their civil rights.

PG-205 (Implementation Report and Inventory) — due within 90 days after the court distributes the guardianship order. A baseline inventory of the ward's physical location, health status, financial assets, bank accounts, benefits, and debts.

PG-210 (Guardianship Annual Report) — due every year, within 30 days of the appointment anniversary. Covers the preceding twelve months of medical care, physical placement, and a detailed accounting of all income and expenses, with bank statements attached.

Missing any of these deadlines can trigger a court inquiry, additional oversight, or even removal as guardian.

The Fee Waiver Form

The standard filing fee for an adult guardianship or conservatorship case is $150. If the petitioner or respondent cannot afford it, submit Form TF-920 (Request for Exemption from Payment of Fees) alongside the petition packet. The court holds the petition until a judge rules on the waiver.

Modification and Review Forms

Guardianship orders are not permanent and unchangeable:

PG-190 (Petition for Review of Guardianship) — the ward, guardian, or any interested party can file this at any time to request modification, reduction, or termination of the guardianship.

PG-106 (Petition for Single Transaction) — for situations where a discrete one-time action is needed (signing a specific contract, accessing emergency funds) without appointing a permanent guardian or conservator.
