Alaska Guardianship Annual Report
The PG-210 Annual Report
Every appointed guardian in Alaska must file Form PG-210 with the Superior Court once a year. The deadline is 30 days after the anniversary of the guardianship order — not the calendar year end, not the ward's birthday, but the date the court signed the appointment order.
The report covers the preceding twelve months and requires detailed information across several categories:
Physical placement — where the ward lives, whether they have moved, and whether the current arrangement meets their needs.
Medical care — a summary of medical treatments, hospitalizations, behavioral health services, and any changes to the ward's condition or functioning.
Financial accounting — a complete record of all income received and expenses paid on the ward's behalf during the reporting period. The court expects bank statements and receipts for major expenditures attached as supporting documentation.
Decision-making participation — how the guardian has encouraged the ward to participate in decisions affecting their own life, as required by Alaska's statutory preference for maximum independence.
The First-Year Reporting Stack
New guardians face a compressed schedule in their first year. Before the annual report, three other filings come due:
PG-120 (Affirmation of Mandatory Education) — within 30 days after the court distributes the guardianship order. Complete at least one hour of training on guardian duties. The Alaska Court System provides this training online.
PG-401 (Guardianship Plan) — within 30 days after the court distributes the guardianship order. A forward-looking plan detailing care arrangements, rights protections, and how the guardian will support the ward's self-determination.
PG-205 (Implementation Report and Inventory) — within 90 days after the court distributes the guardianship order. A baseline inventory of the ward's assets, living situation, health status, and benefit enrollments. This establishes the starting point that future annual reports measure against.
Missing the PG-120 or PG-401 deadlines in the first 30 days is the most common mistake new guardians make. The court will follow up.
The Three-Year Visitor Review
Beyond the annual reports, the court automatically triggers a comprehensive review every three years. A court visitor — an independent evaluator appointed and paid by the state — interviews the ward and the guardian, reviews all annual reports filed since the last review, and submits a formal report to the court.
The visitor's report evaluates whether the guardianship should continue as-is, be reduced in scope, or be terminated entirely. This review mechanism exists because Alaska law presumes that guardianship should be as limited and temporary as possible.

What Happens if You File Late
Late filing triggers escalating consequences. The court may send a reminder notice first, but repeated failures to file can result in a court hearing where the guardian must explain the delay. In serious cases, the court can appoint a new guardian or expand oversight.
Conservators face an additional parallel obligation: Form PG-225 (Conservator's Annual Report) requires a separate financial accounting for estate assets managed under the conservatorship.
