Utah Special Education Transition Checklist — Age 14 Through Adult Services

Parents who fall behind on transition deadlines rarely get a second chance. Schools won't chase you down for a DSPD application, and the Social Security Administration won't remind you about the age-18 redetermination until it's already underway. This checklist puts every critical Utah transition milestone in chronological order so nothing slips.

Age 14 — The Transition IEP Trigger

Utah requires transition planning to begin during the IEP year the student turns 14, two years earlier than the federal IDEA minimum of 16. At this first transition IEP:

  • Confirm that age-appropriate transition assessments (PINS — preferences, interests, needs, strengths) have been completed
  • Review measurable postsecondary goals in education/training and employment (independent living if the team determines it's appropriate)
  • Establish a course of study aligned with postsecondary goals
  • Request that the student be formally invited to attend and participate in the IEP meeting
  • Document all transition components in the written IEP

Ages 15–16 — Agency Connections and Career Exploration

  • Revisit and update transition assessments annually — student preferences change
  • Begin Pre-Employment Transition Services (Pre-ETS) through USOR if not already started (available ages 14–21; students do not need to be active VR clients)
  • Obtain written consent to invite outside agency representatives (USOR, DSPD) to IEP meetings
  • Apply for formal Vocational Rehabilitation (VR) services if the student has employment barriers
  • Start researching DSPD waiver eligibility if the student has an intellectual disability or related condition
  • Review the graduation pathway — regular diploma, alternate diploma, or certificate of completion — and confirm the course of study matches

Age 17 — Pre-Adult Legal Preparations

  • Receive the school's mandatory transfer-of-rights notification (required at least one year before the student turns 18)
  • Research decision-making alternatives: Supported Decision-Making Agreement (SDMA) under H.B. 334, power of attorney, healthcare directive, or guardianship
  • If pursuing guardianship, file the petition at age 17.5 (six months before the 18th birthday) to prevent a gap in legal authority
  • Schedule clinical evaluations if guardianship or DSPD eligibility requires updated psychological testing
  • Gather records: IEP history, transition assessments, medical documentation, therapy reports

Age 18 — The Legal Shift

  • Educational decision-making rights transfer automatically to the student unless a court-ordered guardianship is in place
  • Execute the chosen decision-making framework (SDMA, POA, guardianship order, healthcare directive, HIPAA release)
  • Apply for adult SSI — the age-18 redetermination evaluates the student under adult disability standards without parental income deeming
  • Submit the DSPD intake application through the mySTEPS portal (strict 90-day completion deadline once started)
  • Apply for the Medicaid Work Incentive (MWI) program if the student is working
  • Open an ABLE account if eligible (onset of disability before age 46 as of 2026; standard contribution limit $20,000/year; up to $100,000 disregarded for SSI)

Ages 18–21 — Post-High Transition Programs

  • Enroll in the school district's post-high transition program if the student has not earned a regular diploma
  • Coordinate with the USOR VR counselor to develop an Individualized Plan for Employment (IPE)
  • Track DSPD waitlist status — contact the assigned caseworker annually and report any changes in needs or caregiver capacity to update the NAQ score
  • Continue Pre-ETS services (available through age 21, or 22 if still receiving special education)
  • Monitor the SSI age-18 redetermination outcome and appeal if benefits are denied

Age 21–22 — The School Exit

  • Confirm the student's final year of eligibility under R277-419 pupil accounting rules (birthday between September 2 and December 31: services end at winter recess the year they turn 22; birthday between January 1 and September 1: services end June 30 the year they turn 22)
  • Request the Summary of Performance (SOP) — the school must provide one when eligibility terminates
  • Verify that the SOP documents academic achievement, functional performance, and specific accommodation recommendations
  • Confirm that all adult service applications (DSPD, VR, SSI, MWI) are active before school services end
  • Transition healthcare from pediatric to adult providers

Print it, pin it to the fridge, and review it at every annual IEP meeting. Each milestone triggers specific paperwork, and missing a deadline — especially the DSPD 90-day intake window or the guardianship filing at 17.5 — can mean months of delay.
