SSI Documents Checklist for Arkansas: What You Need for the Application and Age-18 Review

The SSA gives you a packet of forms and a short deadline. What it does not give you is a clear list of everything you should submit alongside those forms. Families who walk in with a complete file get smoother processing. Families who trickle documents in over months risk delays, requests for evidence, and — at the age-18 redetermination — a denial based on incomplete records.

The Core SSA Forms

For a first-time SSI application:

  • SSA-8000 (Application for SSI): The main application establishing financial eligibility — income, resources, living arrangements, and citizenship/immigration status. You can start this at ssa.gov, by phone, or at the local field office.
  • SSA-3368 (Adult Disability Report): The medical side — diagnoses, symptoms, daily functioning, work history, and healthcare providers. The information on this form is used to evaluate whether the applicant meets the adult definition of disability.
  • SSA-827 (Authorization to Disclose Information): Allows the SSA and Arkansas Disability Determination Services to request medical records directly from doctors, hospitals, and schools.

For the age-18 redetermination (if your child already receives SSI):

The SSA initiates this review within one year of the child's 18th birthday. The forms are essentially the same — SSA-3368 and SSA-827 — but the standard changes. The SSA is now evaluating whether your child can engage in any substantial gainful activity under the adult definition, not the childhood standard of "marked and severe functional limitations."

You typically have 30 days from the date on the review notice to submit evidence. If you need more time, tell the SSA before the deadline and ask what extension or other accommodation is available; do not assume an extension is automatic.

Medical Documentation

This is what makes or breaks the case. Assemble these before the appointment:

  • Comprehensive psychological evaluation — IQ testing, adaptive behavior assessment (Vineland-3 or ABAS-3), and functional limitations. If the most recent evaluation is more than two years old, consider getting a new one.
  • Psychiatric or neurological records — diagnostic reports, medication history, treatment notes showing the course of the condition
  • Therapy records — speech, occupational, physical, and behavioral therapy progress notes
  • Hospital records — admissions, ER visits, surgical reports if applicable
  • Physician treatment notes — recent notes from the primary care doctor and any specialists, showing ongoing impairment

The SSA does not count an IEP as medical evidence, but it uses school records to corroborate functional limitations. The more clinical documentation you provide from outside the school system, the stronger the file.

School and Educational Records

  • Most recent IEP with transition goals and accommodations
  • Psychoeducational evaluation from the school (the one used for initial eligibility or triennial reevaluation)
  • Summary of Performance (SOP) if the student is graduating or aging out — request this from the school before exit
  • Report cards and attendance records — the SSA uses these to assess functioning
  • Disciplinary records showing behavioral challenges, if relevant to the disability claim

Financial Documentation

The SSI resource limit is $2,000 for an individual. The SSA will verify:

  • Bank statements — recent statements for all accounts in the applicant's name
  • Proof of living arrangements — lease, room-and-board agreement, or a statement describing who the applicant lives with and whether they receive food and shelter
  • Income documentation — if the applicant works, pay stubs for the last two months; if they receive any other income (DAC, pension, annuity), documentation of the amount
  • ABLE account statements if one is open — the first $100,000 is excluded
  • Vehicle title — one vehicle is excluded; if the applicant owns more than one, the SSA counts the value of additional vehicles

Identity and Citizenship

  • Birth certificate or proof of age
  • Social Security card
  • Photo ID (state ID, passport, or school ID)
  • Proof of U.S. citizenship or eligible immigration status

What to Bring That the SSA Does Not Ask For

The forms ask specific questions. These additional items give the examiner a fuller picture:

  • A written statement from the parent or caregiver describing a typical day — what the individual can and cannot do independently, supervision needs, safety risks
  • Letters from service providers — therapists, job coaches, group home staff, or day program coordinators describing functional limitations they observe
  • A medication list with dosages, prescribing physicians, and side effects that affect daily functioning
  • A communication notebook or behavior log if the individual has significant behavioral challenges — dates, incidents, interventions

After Submission

Once the SSA receives the forms, Arkansas Disability Determination Services conducts the medical evaluation. Processing times vary. If denied, the appeal process begins with a Request for Reconsideration (Form SSA-561) within 60 days.
