The school system is required to serve your child. The adult system is not. That's the core shift families face when their student with a disability exits high school in South Dakota — they're moving from an entitlement-based framework where the school must provide services to an eligibility-based system where funding is capped and waitlists are real.

Understanding the application sequence before the school exit date arrives is what separates families who have a smooth handoff from those who spend months in limbo.

The Entry Point: Dakota at Home

All adult developmental disability services in South Dakota funnel through a single intake point called Dakota at Home. You can start the process by calling 1-833-663-9673 (Option 2) or emailing [email protected] to connect with a Division of Developmental Disabilities intake specialist.

Contact Dakota at Home when the student turns 18 — not when they're about to graduate. Families with students born between April and June face especially tight timelines under the fiscal school year exit rule and need every month of lead time they can get.

The Application Sequence

Step 1 — Submit the DDD Application for Services. The family completes the application and submits it with required clinical documentation, including a psychological evaluation from a licensed psychologist. This evaluation must demonstrate that the applicant meets an Intermediate Care Facility level of care (ICF/IID standard).

Step 2 — The ICAP Assessment. Within seven days of receiving a completed application, the DDD contacts the family to schedule the Inventory for Client and Agency Planning assessment. The ICAP measures adaptive behavior and functional limitations across motor, social, communication, and daily-living domains. The results determine both program eligibility and funding levels.

Step 3 — Eligibility Decision. The DDD must issue a formal eligibility determination within 45 days of receiving a completed application. If approved, the family is connected with a Family Support Coordinator or Case Manager depending on which waiver they're entering. If denied, the applicant has 30 days to file a formal appeal.

Step 4 — Financial Medicaid Application. Once programmatic eligibility is established, the coordinator helps the family complete the adult Medicaid application through the Department of Social Services. Only the student's individual income and assets are evaluated — parental income is not counted.

The Waitlist Reality

Eligibility doesn't guarantee immediate services. South Dakota maintains an active interest list for its developmental disability waivers. As of recent reporting, approximately 185 individuals are waiting, and wait times range from 1 to 2 years. The DDD prioritizes individuals in crisis — those whose health and safety are in immediate jeopardy or who face imminent risk of institutionalization.

Services are delivered through regional Community Support Providers, not a centralized state agency, so availability varies by county. Families in rural parts of the state should contact their regional provider during the student's 17th year to begin early evaluations.

What Comes After Eligibility

Once a waiver slot opens, the family chooses between two program tracks: the CHOICES Waiver for comprehensive supports including residential services and day programs, or the Family Support 360 Waiver for self-directed in-home care.
