New Mexico Medicaid Renewal for Disability Under Turquoise Care

The Renewal Shift Coming in 2027

Starting January 1, 2027, New Mexico's Health Care Authority will implement mandatory work and activity requirements for certain adult Medicaid beneficiaries aged 19 to 64. Non-exempt adults must complete and document at least 80 hours per month of qualifying activities — paid employment, volunteering, community service, job training, or half-time school attendance.

Alongside the work requirements, Medicaid renewal periods for affected individuals shorten from 12 months to 6 months. That means twice as many renewal cycles per year, each requiring updated documentation.

For families of adults with disabilities, the critical question is whether these changes apply to their situation. The short answer: they shouldn't, but the system doesn't always know that automatically.

The Disability Exemption and the ASPEN Problem

Adults who have a documented physical or mental disability, or who are active SSI recipients, are explicitly exempt from the work requirements. SSI recipients in particular should be automatically flagged — New Mexico is a Section 1634 state, meaning SSA transmits eligibility data directly to the Health Care Authority.

The problem is operational. The state's ASPEN computer system, which processes Medicaid eligibility, does not always apply the disability exemption automatically. If the system can't verify the exemption through its data match with SSA, the Health Care Authority will issue a 30-day notice of noncompliance. Failure to respond or submit proof of the disability exemption within those 30 days results in immediate termination of Medicaid coverage.

This isn't a theoretical risk. The handoff between SSA's federal files and the state's ASPEN system has historically produced gaps, particularly for individuals whose SSI status changed recently (new approvals, redeterminations, or reinstatements after appeal).

How to Protect Coverage at Each Renewal

Confirm the exemption is on file. Before the first renewal cycle, log into YES New Mexico (yes.nm.gov) and verify that the individual's disability or SSI exemption status is accurately recorded. If the file shows no exemption or the status is unclear, contact the HCA Consolidated Customer Service Center to manually push the update. Don't wait for the renewal notice to discover the flag is missing.

Respond to every renewal notice. Even if the exemption should handle everything automatically, treat each renewal packet as a deadline. Open it, verify the information, and return it completed. Failure to respond or submit proof within 30 days can result in immediate termination of Medicaid coverage.

Keep SSI documentation accessible. The fastest way to resolve an ASPEN verification failure is to submit current proof of SSI receipt. A recent SSI award letter, benefit verification letter (available through my Social Security at ssa.gov), or a printout showing active SSI status works. Having this ready before the renewal cycle starts saves the scramble when a noncompliance notice arrives.

Track the renewal calendar. Under the 6-month cycle, renewals come twice a year. Mark both dates. The Health Care Authority mails renewal packets before the due date, but relying on mail delivery alone adds unnecessary risk.

What Happens If Coverage Lapses

If Medicaid terminates due to a missed renewal or unresolved exemption verification, the individual loses access to all Turquoise Care MCO services — including any DD or Mi Via waiver services that run through the Medicaid system. Waiver services cannot continue without active Medicaid eligibility.

Reinstatement requires reapplying through YES New Mexico and re-establishing eligibility, which can take weeks. During that gap, prescriptions, therapy appointments, and direct support services are unfunded.
