New Mexico Medicaid After SSI Denied at 18: Turquoise Care Options

When your child's SSI gets denied at the age-18 redetermination, the first fear isn't the cash benefit — it's the Medicaid coverage. In New Mexico, SSI and Medicaid are linked through a federal agreement called Section 1634: approval for SSI automatically means Medicaid eligibility, no separate application needed. But the reverse is also true — lose SSI, and that automatic Medicaid enrollment is at risk.

The situation isn't as dire as it sounds. New Mexico has several pathways to maintain health coverage even after an SSI denial.

How the SSI-Medicaid Link Works in New Mexico

New Mexico is a Section 1634 state. When SSA approves an SSI application, it transmits the eligibility file directly to the New Mexico Health Care Authority, which enrolls the individual in Turquoise Care — the state's Medicaid managed care program that replaced Centennial Care in July 2024.

The member selects from four managed care organizations: Blue Cross Blue Shield of New Mexico, Presbyterian Health Plan, Molina Healthcare of New Mexico, or UnitedHealthcare Community Plan. If no selection is made within the enrollment window, the state auto-assigns a plan.

When SSI stops, this automatic pathway closes. But Medicaid eligibility doesn't necessarily end — it just requires a different category.

Extended Medicaid After SSI Termination

New Mexico provides a continuation period after SSI benefits end. If SSI is terminated due to the age-18 redetermination, the individual keeps Medicaid coverage for up to two months while the family explores alternative eligibility categories.

During this window, families should apply under one of the following pathways.

Alternative Medicaid Categories for Adults with Disabilities

ABD Medicaid (Aged, Blind, and Disabled): If the individual meets the state's disability criteria independently of SSI — which uses the same medical standard but allows the state to make its own determination — they can qualify for Medicaid through this category. The income and resource limits are strict ($2,000 resource limit), but the disability determination is separate from SSA's.

Working Disabled Individuals (WDI) — Category 074: For young adults who are working but have significant disabilities, this Medicaid buy-in program allows earned income well above standard Medicaid limits while maintaining full coverage. Many participants pay no monthly premium, depending on their income bracket relative to the Federal Poverty Level. Application goes through HCA's online portal, YES New Mexico (yes.nm.gov), with proof of active employment and ongoing disability status.

DD or Mi Via Waiver Medicaid: If the individual has a developmental disability and is on track for a DD Waiver or Mi Via Waiver allocation, the waiver application process includes its own institutional Medicaid pathway. This requires a specialized Institutional Care Medicaid Application submitted to the Income Support Division's Institutional Care Waiver Unit in Bernalillo — separate from standard Medicaid.

The 2027 Work Requirements and Disability Exemptions

Starting January 1, 2027, New Mexico's Medicaid framework introduces mandatory work and activity requirements for certain adult beneficiaries aged 19 to 64. Non-exempt adults must document at least 80 hours per month of qualifying activities — paid employment, volunteering, job training, or half-time school attendance. Renewal periods for affected individuals shorten from 12 months to 6 months.

The critical detail: individuals with a documented disability or who receive SSI are explicitly exempt from these requirements.

But the exemption isn't always applied automatically by the state's ASPEN computer system. Families must actively confirm that the disability exemption is recorded in the young adult's YESNM.gov file. If HCA can't verify the exemption at renewal, they'll issue a 30-day noncompliance notice. Failing to respond with proof of the disability exemption within 30 days results in Medicaid termination.

This administrative trap catches families who assume the exemption is automatic. Check the file proactively — don't wait for the noncompliance letter.

SSI-Linked Medicaid Eligibility

If your young adult maintains SSI (or wins on appeal), New Mexico's Section 1634 linkage keeps Medicaid eligibility connected to SSI, with Turquoise Care coverage.

For those who lose SSI but qualify under another disability-related Medicaid category, the managed care enrollment and MCO selection process is the same — the coverage is functionally identical through Turquoise Care, just administered under a different eligibility code.

Filing an SSI Appeal Preserves Medicaid

If you appeal the SSI denial within 10 days of receiving the cessation notice, both SSI payments and Medicaid continue during the appeal. This is the strongest protection available — it buys time without any gap in coverage.
