Why MassHealth Coverage Changes at 18
If your child has been covered under MassHealth as a dependent, their eligibility is re-evaluated when they turn 18. As a legal adult, they are no longer eligible under family-based categories. They need their own MassHealth enrollment — and if the basis is disability rather than income alone, the application process involves significantly more paperwork than most families expect.
The goal is MassHealth Standard or MassHealth CommonHealth. Standard provides comprehensive coverage with no premiums for adults who meet income limits (133% of the federal poverty level). CommonHealth extends coverage to disabled adults whose income exceeds that threshold, in exchange for a sliding-scale premium.
The Dual-Form Requirement
MassHealth disability applications at age 18 require two separate submissions that work in tandem:
1. The MassHealth Application (Form ACA-3). This is the standard health coverage application. It establishes basic demographics, residency, citizenship, and household information. You can submit it online through the MA Health Connector or by paper.
2. The MassHealth Adult Disability Supplement. This is the form that most families either miss or underestimate. It is a detailed clinical questionnaire about the individual's functional limitations — how the disability affects their ability to work, manage daily tasks, and live independently. It must be submitted in addition to the main application.
If the individual already receives SSI, their disability is considered confirmed by SSA and MassHealth typically grants coverage without requiring the supplement. But if the SSI redetermination is still pending or was denied, the supplement becomes the primary vehicle for establishing disability with MassHealth.
Completing the MADS-MR Release Forms
Here is where the process gets administratively heavy. The Adult Disability Supplement requires signed MADS-MR (Massachusetts Adult Disability Supplement — Medical Release) forms authorizing MassHealth's Disability Evaluation Services to obtain medical records directly from the individual's providers.
You need a separate signed release form for each medical provider, therapist, psychiatrist, psychologist, and hospital that you list in the supplement. The supplement includes five copies, so families with more than five listed providers need additional copies.
Each form must include the provider's full name, practice address, and phone number. Incomplete or unsigned forms will stall the review.

Where to Send the Package
The completed Adult Disability Supplement and all MADS-MR release forms get mailed to the Disability Evaluation Services (DES) office in Worcester — not to MassHealth's general enrollment center and not to your local MassHealth office. The main ACA-3 application goes to the Health Connector or MassHealth Enrollment Center separately.
This split in mailing destinations confuses many families. The main application and the disability supplement travel different paths and are reviewed by different teams, but both must be submitted to establish disability-based MassHealth coverage.
Timing the Application
The optimal timing is to submit both forms immediately after the 18th birthday. If you wait for the SSI age-18 redetermination result before applying for MassHealth, you risk a gap in coverage — SSI decisions can take months, and MassHealth applications also take weeks to process.
Filing both simultaneously gives you a backup: if SSI is approved, MassHealth coverage follows automatically. If SSI is denied, the Adult Disability Supplement creates an independent path to MassHealth disability coverage through the state's own evaluation process.
Responding to the Blue Envelope
Once enrolled in MassHealth, the individual may receive annual renewal notices in a distinctive blue envelope. Respond by the due date listed in the renewal notice; failing to respond can result in coverage termination. If coverage closes for failure to respond to a renewal, a 90-day reconsideration period may allow reinstatement, but it is not the renewal response deadline and does not apply to every type of missing information.
Mark the calendar for the annual redetermination period and keep clinical documentation updated. An organized file makes the renewal almost routine; a disorganized one turns it into a crisis.
