What Iowa State Supplementary Assistance Covers

Federal SSI pays a maximum of $994 per month in 2026 for an eligible individual. For many adults with disabilities in Iowa, that amount doesn't cover their actual living costs — especially if they live in a certified family-life home or a residential care facility. Iowa's State Supplementary Assistance (SSA) program fills that gap with 100% state-funded payments on top of the federal SSI benefit.

The program isn't well-known. Iowa HHS administers it separately from Medicaid and separately from the HCBS waiver system, and it has its own eligibility rules. But for families in the right circumstances, it can add several hundred dollars per month to an adult child's total benefit package.

Who Qualifies

To receive State Supplementary Assistance, the individual must meet Social Security's definition of disability (aged, blind, or disabled), be an Iowa resident, and have countable resources at or below the $2,000 individual limit — the same threshold as SSI. Qualifying for SSA automatically enrolls the individual in Iowa Medicaid, which means this program creates a separate Medicaid pathway independent of the Section 1634 SSI-Medicaid link.

The Three SSA Categories

Iowa's program pays different amounts depending on the individual's living arrangement:

Family-Life Home Assistance is designed for individuals who live in a certified family-life home — a private home where a non-relative caregiver provides 24-hour supervision and personal care. The maximum monthly payment to the family-life home provider is $1,026, plus the individual receives a $130 personal needs allowance. The total payment standard is $1,156 per month.

Dependent Person Allowance provides a supplement when an eligible client supports a dependent relative. The maximum supplement payment is $518 per month. For an aged or disabled client with a dependent relative, the combined standard is $1,512 per month. For a blind client with a dependent relative, it's $1,534.

Residential Care Facility Assistance covers individuals who require care in a licensed RCF. The flat per diem rate is $17.86 per day, with a maximum cost-related per diem of $38.47. Residents receive a $130 monthly personal needs allowance.

How SSA Interacts with Other Benefits

State Supplementary Assistance sits on top of SSI — it doesn't replace it. An individual can receive both SSI ($994/month maximum) and SSA simultaneously. The SSA payment is calculated based on the difference between the individual's living costs in their specific arrangement and their available income from SSI and other sources.

Because SSA triggers automatic Medicaid enrollment in Iowa, it creates a safety net for individuals who lose SSI eligibility during an age-18 redetermination but might qualify for state supplement based on their living arrangement. It's a pathway families often overlook.

The $2,000 resource limit applies to SSA just as it does to SSI. Assets in an IAble account are excluded up to $100,000 for SSI purposes, and Medicaid disregards the IAble balance up to the plan's $505,000 maximum. A properly funded ABLE account or special needs trust keeps the individual under the resource ceiling while preserving savings.

How to Apply

Applications for State Supplementary Assistance go through Iowa HHS, not the Social Security Administration. Contact your local Iowa HHS office and request Form 470-5170 (Application for Health Coverage and Help Paying Costs).
