Health First Colorado After SSI Turns 18

Before your child turns 18, their Health First Colorado coverage probably rides on your household's Medicaid case. At 18, that changes. The state treats your child as an independent adult — their own household of one — and the eligibility rules shift accordingly. If SSI is active, Medicaid usually continues automatically. If SSI stops or gets denied at the age-18 redetermination, the automatic link breaks, and you need a backup plan.

The Automatic Pathway: SSI Keeps Medicaid Running

Colorado is a Section 1634 state, which means that anyone receiving SSI cash payments automatically qualifies for Health First Colorado without a separate Medicaid application. As long as the monthly SSI check is flowing, Medicaid enrollment continues.

This is the simplest scenario: your child turns 18, passes the SSI age-18 redetermination under the adult standard, keeps getting SSI, and Health First Colorado continues without interruption.

When the Automatic Link Breaks

The link breaks in three common situations:

SSI denial at redetermination. The SSA applies adult disability criteria and denies. Without SSI cash, the automatic Medicaid pathway shuts off. You'll need to file a separate Medicaid application through Colorado PEAK.

SSI suspension for excess resources. Your child has a savings account with more than $2,000, or someone deposited gift money that pushed them over the limit. SSI suspends. The automatic Medicaid link may end, so file a standalone application.

DAC benefits replace SSI. When a parent retires, becomes disabled, or dies, the DAC payment typically exceeds SSI limits. The Section 1634(c) disregard is supposed to protect Medicaid here, but it requires that SSI was active immediately before DAC started.

Filing a Separate Adult Medicaid Application

If the automatic pathway isn't available, you apply for Health First Colorado as a disabled adult through the PEAK portal or your county Department of Human Services. Colorado has several Medicaid categories that can apply:

• SSI-related Medicaid — for individuals whose income and resources fall within SSI limits even if they're not receiving SSI cash
• Medicaid Buy-In for Working Adults with Disabilities (WAwD) — if your adult child is working at least one hour per month and has countable income after disregards at or below 450% of the Federal Poverty Level ($5,985/month in 2026), with no asset limit
• Standard Medicaid — income-based eligibility under the Affordable Care Act expansion

The disability determination is handled either by the SSA (if there's a pending or approved SSI claim) or by Colorado's State Disability Determination vendor (if the individual has never applied for SSI). The state-level determination uses the Med-9 form, available through HCPF.

The Annual Renewal

Health First Colorado renewals happen once a year regardless of pathway. The county sends a renewal packet, and the adult must respond with updated income, asset, and disability information. Missing the renewal deadline can result in termination — even when nothing about eligibility has changed.

Set a calendar reminder 30 days before the renewal date. If the individual's disability status hasn't changed and their income remains within limits, the renewal is straightforward. If employment status has changed, switching to or from the WAwD pathway may be the right move.
