Medi-Cal After SSI Turns 18

The Automatic Link Between SSI and Medi-Cal

In California, anyone receiving at least $1 in monthly SSI cash benefits automatically gets free, full-scope Medi-Cal. No separate application, no enrollment form, no county interview. The data flows directly from the Social Security Administration to the county welfare department through a federal interface. This is the Section 1634 linkage, and it's the easiest path into Medi-Cal for adults with disabilities.

When your child turns 18 and the SSI age-18 redetermination happens, this automatic linkage is what's at stake. If SSI continues, Medi-Cal continues. If SSI stops, the automatic connection breaks — but that doesn't mean Medi-Cal coverage has to end.

What Happens If SSI Stops at 18

When the SSA issues a medical cessation notice (determining the young adult no longer qualifies under the adult disability standard), the automatic Medi-Cal link is at risk, but benefit continuation can preserve linked Medi-Cal during the appeal. If SSI actually stops, the automatic connection breaks — but California law requires the county to act before coverage lapses.

Under SB 87, the county welfare department must conduct a review to determine whether the individual qualifies for Medi-Cal through a different pathway. This review is supposed to happen proactively — the county should evaluate alternative eligibility before the SSI-linked coverage terminates.

In practice, counties don't always move fast enough. Families who receive an SSI cessation notice should contact their county social services office immediately rather than waiting for the review to happen on its own.

The Main Alternative Pathways

If SSI stops, there are several Medi-Cal categories that can maintain coverage:

Aged & Disabled Federal Poverty Level (A&D FPL) — This is the most common fallback for adults with disabilities who lose SSI. To qualify, the individual must meet Social Security's definition of disability and have countable monthly income below $1,856 for an individual ($2,510 for a couple) in 2026. The countable asset limit is $130,000 through June 2027, dropping to $21,000 after that.

For a young adult who lost SSI because of the medical cessation — not because of income — and has minimal personal income, A&D FPL is usually the clearest path.

Medi-Cal with a Share of Cost (Medically Needy ABD-MN) — If income exceeds the A&D FPL threshold, this category kicks in. There's no upper income limit, but the individual must pay a specified monthly amount of medical expenses out-of-pocket before Medi-Cal covers anything in that month. The asset limit is the same $130,000 through June 2027.

250% Working Disabled Program — If the young adult is working, this program provides Medi-Cal for individuals whose net family income is under 250% of the federal poverty level. The key advantage: all unearned disability income is fully excluded from the income calculation. The asset limit is $130,000 in 2026.

The Deemed SSI Safety Net

There are situations where Medi-Cal continues through the original SSI-linked pathway even after SSI payments drop to zero. The most common scenario: the young adult starts receiving Disabled Adult Child (DAC/CDB) benefits on a parent's work record, and the DAC payment pushes their income above SSI eligibility. Under Section 1634's Deemed SSI rules, these individuals keep free Medi-Cal as if they were still receiving SSI.

The Deemed SSI protection also applies to Pickle Amendment beneficiaries and Disabled Widow(er)s. These groups remain exempt from the reinstated Medi-Cal asset limits — they follow only the federal SSI resource standard of $2,000.

The 2026 Asset Limit Reinstatement

After eliminating asset limits entirely in 2024 and 2025, California reinstated them for non-MAGI Medi-Cal programs effective January 1, 2026, under Assembly Bill 116. Current enrollees don't need to verify assets immediately — verification happens during the next scheduled annual redetermination.

For a young adult turning 18 in 2026 or 2027, the practical impact depends on whether they hold assets in their own name. Most don't — but if a well-meaning relative set up a savings account or made deposits, those funds count. The $130,000 limit through June 2027 is generous enough that few families will hit it, but the drop to $21,000 in July 2027 requires advance planning.

A CalABLE account excludes up to $100,000 from the SSI resource count, and excess funds beyond that can be sheltered in a properly structured Special Needs Trust. Both strategies should be in place before the 2027 cliff.

Privacy and Medical Decision-Making at 18

A less obvious Medi-Cal change at 18: under HIPAA and California law, the young adult's medical privacy rights are fully activated. Providers cannot share medical records or discuss treatment plans with parents unless the young adult has executed a Medical Power of Attorney, a Supported Decision-Making Agreement, or a court has granted a Limited Conservatorship.

This catches families during Medi-Cal renewals specifically. The county may send renewal forms to the young adult, not the parent. If the young adult can't manage the paperwork independently and no legal authority is in place, renewals can lapse simply because nobody responded.
