The Pathways to Possibilities Secondary Transition Toolkit is Colorado's official state-produced resource for special education transition planning. Published by CDE's Exceptional Student Services unit, it's comprehensive, free, and available to anyone.

It's also written primarily for compliance officers, transition coordinators, and IEP team members — not for parents trying to figure out what to do next Tuesday.

That's not a criticism of the toolkit. CDE built it to serve the professionals who implement transition services across hundreds of school districts. It does that job well. But families who download it expecting a step-by-step action plan tend to find themselves deeper in jargon and further from clarity.

What the Toolkit Actually Contains

The Pathways to Possibilities toolkit covers the structural requirements for secondary transition under Colorado's Exceptional Children's Educational Act:

More specifically, the toolkit doesn't integrate:

These aren't CDE's responsibility — they belong to HCPF, the Social Security Administration, the courts, and healthcare providers, respectively. But families have to navigate all of them simultaneously, and no single state resource brings them together.
