West Virginia Medicaid and the 1634 Agreement: Automatic Enrollment, Renewals, and Work Requirements

What Being a 1634 State Means

West Virginia has a Section 1634 agreement with the Social Security Administration. The practical effect is simple: when SSA approves your adult child for SSI, the data exchange between SSA and the West Virginia Department of Human Services (DoHS) automatically enrolls them in Medicaid. No separate state application is required.

This is a significant administrative advantage. In non-1634 states, families must file a separate Medicaid application after SSI approval, which can create a gap in coverage lasting weeks or months. In West Virginia, coverage is automatic once the SSI-DoHS data exchange processes the approval, but families should still verify the status.

The flip side is equally important. If SSA denies SSI at the age-18 redetermination, that denial flows through the same data exchange and terminates Medicaid. This is why the 10-day benefit continuation rule during appeals is critical — filing the appeal request within 10 days preserves both the SSI check and the automatic Medicaid coverage until the appeal is resolved.

Verifying the Medicaid Link

After SSI approval, verify that the automatic Medicaid enrollment actually processed by checking the WV PATH portal (wvpath.wv.gov). The electronic data exchange between SSA and DoHS occasionally experiences delays, and a missing record can leave your adult child without a Medicaid card even though they are technically eligible.

If the WV PATH portal does not show active Medicaid coverage, contact DoHS Client Services at 1-877-716-1212 with the SSI approval letter and the Social Security number. The caseworker can verify the data exchange and explain the next enrollment step.

The Annual Renewal Process

Medicaid coverage is not permanent once activated. West Virginia requires periodic eligibility renewals, typically annual. DoHS mails a renewal packet to the address on file. The family must complete and return the packet by the deadline — failure to respond results in coverage termination, even if nothing has changed.

For SSI-linked Medicaid, the renewal is usually administrative: DoHS verifies that SSI is still active, and coverage continues automatically. But if the mailing address is outdated (common when young adults move out of the family home), the renewal letter goes undelivered and coverage lapses silently. Medicaid Work Requirements and the Disability Exemption

Under the community engagement requirements enacted through H.R. 1 ("The One Big Beautiful Bill Act") in July 2025, non-exempt Medicaid expansion members must complete at least 80 hours per month of work, school, or volunteering.

Individuals who are blind, disabled, or have a severe physical or mental impairment that limits daily activities are explicitly exempt from these requirements. The exemption applies to anyone receiving SSI-based Medicaid through the 1634 automatic enrollment — your adult child's SSI eligibility is itself documentation of the qualifying disability.

This is why maintaining categorical SSI-related Medicaid (through the 1634 link) is preferable to Medicaid expansion coverage, even when both might technically apply. How to Apply Separately When Needed

If your adult child does not qualify for SSI — perhaps because they have too much earned income but still meet the medical disability standard — they can apply for Medicaid directly through the WV PATH portal.
