North Carolina State Special Assistance Program for Disabled Adults
North Carolina doesn't have a traditional state SSI supplement that adds a flat amount to every recipient's monthly check. Instead, the state runs the Special Assistance (SA) and Special Assistance In-Home (SAIH) programs — cash supplements for low-income disabled or elderly residents that can significantly exceed what a generic state supplement would provide.
The distinction matters because SA isn't automatic. You have to apply separately, meet specific living-arrangement requirements, and submit a physician's FL-2 form. Parents navigating the adult benefits transition often don't know it exists.
Two Models, Two Different Living Arrangements
Special Assistance for Adult Care Homes (SA/ACH) pays a monthly supplement directly to licensed residential facilities — adult care homes, family care homes, and group homes — to cover room and board costs. The facility must agree to accept the state-approved rate.
Special Assistance In-Home (SAIH) pays a monthly cash check directly to eligible individuals who live in their own home (or a family member's home). A county case manager assesses whether the person can safely remain at home with the combination of SAIH funding and community supports.
SAIH is the pathway most relevant to families transitioning a young adult to adulthood who plans to continue living at home.
2026 Rates
The North Carolina General Assembly sets the rates annually:
- Basic Rate: $1,397/month, establishing a maintenance amount of $1,467/month
- Enhanced Rate: $1,792/month (for individuals with a verified Alzheimer's or related dementia diagnosis), establishing a maintenance amount of $1,862/month
The actual monthly supplement is calculated by subtracting the individual's countable income from the approved maintenance limit. If a young adult receives $994/month in federal SSI, the state-county supplement covers the remaining balance up to the maintenance limit, including a designated $70 personal needs allowance.
Eligibility Requirements
- Age: Must be at least 18 with a verified Social Security disability determination, or at least 65
- FL-2 Form: A physician must complete this form certifying the applicant needs the level of care provided by an adult care home — even for SAIH applicants who will stay at home
- Assets: Countable assets capped at $2,000 for an individual (consistent with SSI limits). Spousal assets are completely disregarded
- Automatic Medicaid: Eligibility for Special Assistance automatically qualifies the recipient for full Medicaid coverage — another pathway into Medicaid for young adults who may have lost SSI

How to Apply
Applications go through the local county Department of Social Services. The county DSS processes the FL-2 medical evaluation and the financial eligibility determination. Processing times vary by county, but the FL-2 form is the bottleneck — schedule the physician appointment early.
For young adults receiving SSI, the SA application should be filed as soon as the living arrangement is established. If the young adult will live in a group home, the facility's administrator typically coordinates the SA/ACH application. If they'll remain at home, the family initiates the SAIH application through county DSS.
Why This Matters for the Transition
The SA program fills a gap that SSI alone cannot cover. Federal SSI at $994/month doesn't come close to covering the actual cost of room, board, and personal care in most North Carolina communities. SA effectively creates a state-funded bridge between what SSI provides and what it actually costs to support a disabled adult in either a residential facility or their family home.
