The SSI age-18 redetermination packet is the most evidence-intensive filing most Nevada families will ever handle. The SSA evaluates your child under adult disability standards for the first time, and the strength of the evidence file directly determines whether benefits continue. Gathering records before the review packet arrives gives you a head start.

What the SSA Is Looking For

The adult disability determination follows a five-step sequential evaluation. Nevada's Disability Determination Services — not the SSA itself — conducts the medical review. At its core, the examiner needs to answer one question: can this individual perform any type of substantial gainful activity in the national economy?

The evidence must show not just that a diagnosis exists, but how it limits function. A letter that says "patient has autism spectrum disorder" is less useful than one that says "patient cannot follow multi-step verbal instructions, requires prompting for personal hygiene, and becomes physically aggressive in unstructured environments."

Medical Records

These are the foundation. Request records from every provider who has treated your child in the past two years:

• Primary care physician — general health, medications, appointment history
• Psychiatrist or psychologist — diagnosis, treatment plan, functional observations, psychological testing scores (IQ testing and adaptive behavior assessments are especially powerful)
• Neurologist — for epilepsy, traumatic brain injury, or other neurological conditions
• Speech-language pathologist — communication limitations, receptive and expressive language levels
• Occupational therapist — fine motor limitations, sensory processing difficulties, daily living skill assessments
• Behavioral analyst (BCBA) — functional behavior assessments, behavior intervention plan data, frequency/severity of challenging behaviors

Request records in writing and give providers at least two weeks. Many Nevada providers charge a copying fee — budget $25–$50 per office.

School Records

IEP documentation is often the strongest evidence families have, because it contains years of objective, third-party data about how the disability affects daily functioning:

• Most recent IEP — goals, accommodations, modifications, present levels of performance, and transition assessments
• Triennial evaluation — psychoeducational testing results, including cognitive assessments and adaptive behavior scales
• Progress reports — objective data on goal achievement (or lack of it) over time
• Teacher narrative statements — ask teachers to write a brief statement describing classroom accommodations, behavioral incidents, and the level of support your child requires
• Transition assessments — vocational evaluations, independent living assessments, and community-based assessment results
• Discipline records — if relevant, manifestation determinations and behavior incident reports document the severity of behavioral challenges

Nevada school districts must provide parents access to IEP records. Ask the district about its copying procedures and any fees, and submit a written records request to your child's special education case manager.

Third-Party Function Reports

The SSA values observations from people who see your child regularly in non-clinical settings. Ask family members, caregivers, respite providers, church leaders, or job coaches to complete a third-party function report describing:

• What your child can and cannot do independently (dressing, cooking, managing money, using transportation)
• How the disability affects social interactions
• How the individual handles changes in routine or unexpected situations
• What supervision is required and how many hours per day

Be specific. "He needs help" is vague. "He cannot prepare a meal without burning himself and must be supervised in the kitchen at all times" tells the examiner something they can use.

Financial Documentation

Because parental deeming ends at 18, the SSA recalculates eligibility based solely on your child's personal income and resources. Have ready:

• Bank statements showing the child's accounts (must be under $2,000 total)
• Any earned income records (pay stubs, W-2s)
• Documentation of any other income (child support, annuities, trust distributions)

Organizing the File

Don't submit a box of loose papers. Organize records chronologically within each category, use tabs or section dividers, and create a cover sheet listing every document included with dates. Nevada Disability Determination Services examiners review hundreds of files — a well-organized submission gets a more thorough review.
