Kansas Guardianship Annual Report: What to File and When
Every guardian and conservator in Kansas must file annual reports with the District Court. Under the KUGCOPAA reforms that took effect in January 2026, these reports are not optional paperwork — they are the court's primary tool for supervising guardianship arrangements and protecting the individual's rights.
What Guardians Must File
Guardians file the "Report of the Guardian of the Child or Adult Subject to Guardianship with Annual Financial Report." This form covers two areas:
Personal status reporting. Where the individual lives, their physical and mental health, any changes in their condition, medical care received during the reporting period, and how the guardian has supported the individual's autonomy and participation in decisions.
Financial reporting. If the guardian manages any personal funds (below the statutory threshold that would require a conservator), the report includes a simplified financial accounting of income received, expenditures made, and remaining balances.
Conservators file a separate, more detailed form: the "Conservator's Report and Annual Accounting." This covers all income, expenditures, investments, and transactions for the protected person's estate. Conservators must also maintain an adequate fiduciary bond to protect estate assets — the court reviews bond adequacy as part of the annual report review.
Where to Get the Forms
The Kansas Judicial Council publishes both reporting forms on their website under Guardianships, Conservatorships, and Other Protective Arrangements. The forms were updated in 2026 to reflect KUGCOPAA requirements — older versions that use terms like "ward" will be rejected by the clerk.
Filing Deadlines and Consequences
Annual reports are required each year. Check the court order and your county's clerk of the court for the exact filing deadline.
Missing an annual report filing is taken seriously. The court can:
- Issue a show-cause order requiring the guardian to appear and explain the delay
- Appoint a court liaison to investigate the guardianship
- Modify or restrict the guardian's authority
- Remove the guardian entirely if the failure reflects a pattern of neglect or noncompliance
The court's review of annual reports is where the Guardianship Plan accountability lives. Judges compare the report against the approved plan to verify the guardian is acting within their authorized scope and following the substituted judgment standard.

Common Mistakes That Trigger Court Scrutiny
Vague health updates. Writing "individual is doing well" does not satisfy the reporting requirement. The court wants specifics: which providers were seen, what treatments occurred, how the individual's condition changed.
Missing autonomy documentation. Under KUGCOPAA, guardians must demonstrate how they are supporting the individual's self-determination. Reports that show no evidence of consulting the individual on decisions, maintaining their social connections, or involving them in their own care draw court attention.
Commingled finances. If you manage personal funds, they must be tracked separately from your own finances. The report must show a clear accounting trail. Conservators face even stricter requirements — SSI benefits managed through a representative payee must be kept in a separate, properly titled account and reported to the SSA under federal rules. Follow the court's instructions for any related conservatorship accounting.
