Diabetes Medical Management Plan: What Schools Need from Your Child's Doctor

The endocrinologist hands you a stack of orders — insulin-to-carb ratios, correction factors, target glucose ranges, glucagon protocols. The school needs all of it translated into something a teacher, a cafeteria aide, and a bus driver can follow. That document is the Diabetes Medical Management Plan.

What a DMMP Contains

A DMMP is a physician-signed medical order set that gives school staff the clinical parameters they need to manage your child's type 1 diabetes during the school day. A complete DMMP covers:

  • Blood glucose targets — the specific range (often 80–180 mg/dL) your child's endocrinologist considers safe, along with the thresholds that trigger intervention
  • Insulin-to-carbohydrate ratios — how many grams of carbohydrate one unit of insulin covers, which can vary by meal and time of day
  • Correction factor — how much one unit of insulin is expected to lower blood glucose above the target range
  • CGM and pump settings — which alerts to enable, what the automated insulin delivery system handles on its own, and when staff must intervene manually
  • Glucagon orders — the route (intranasal Baqsimi or injectable Gvoke/Zegalogue), the dose, and the specific symptoms that require administration
  • Ketone monitoring protocol — when to check for ketones (typically at blood glucose ≥240 mg/dL), what action to take based on results, and when to call 911

The treating endocrinologist writes the DMMP. The school nurse or health services team translates it into daily operational procedures.

Why a DMMP Alone Is Not Enough

A DMMP is a clinical document that sets out medical orders. By itself, it does not identify the school's Section 504 placement or procedural safeguards; the school may still have legal duties to provide services the student needs to access education.

An Individual Health Plan built from the DMMP is a clinical and operational document. By itself, it does not provide Section 504's procedural safeguards, although disability law or district policy may provide other ways to challenge a failure to deliver required services.

A Section 504 plan documents the educational services and accommodations the school must provide; the DMMP supplies the clinical orders those supports follow. The plan can incorporate or reference the DMMP so staff have the current instructions. Section 504 requires notice of actions regarding identification, evaluation, or placement and provides an impartial hearing to challenge those actions.

Getting the DMMP into a 504 Plan

The process is straightforward:

  1. Get the DMMP signed and current. The endocrinologist should update it at least annually and whenever insulin regimens, target ranges, or devices change.
  2. Submit a written 504 evaluation request to the school principal and district 504 coordinator. Attach the DMMP.
  3. At the 504 meeting, make sure every DMMP order maps to a specific accommodation with imperative language. "School staff shall check blood glucose before physical education" is enforceable. "Staff should check glucose when possible" is not.
  4. Name trained staff by role. The plan should designate at least three trained diabetes care personnel — a primary responder and two backups — and identify the tasks they may perform under applicable state law and the DMMP. In California, Torlakson permits trained, voluntary unlicensed staff to administer insulin under written physician orders and with parental consent.

What to Include Beyond the Clinical Orders

The DMMP covers the medical parameters. The 504 plan covers the rest of the school day:

  • Unrestricted CGM smartphone access, exempt from cell phone policies
  • Permission to eat, drink, and use the bathroom without asking
  • Stop-the-clock testing breaks during hypo- or hyperglycemia
  • Trained personnel on every field trip and school bus route
  • A 72-hour emergency diabetes supply kit in the classroom for lockdowns
  • Substitute teacher protocols with a red-flag summary sheet in the sub folder
