Apple Health Enrollment Washington — Every Disability Pathway to Medicaid

The SSI Automatic Pathway

Washington is a Section 1634 state, which means the Social Security Administration handles initial Medicaid determinations for SSI recipients. When SSA approves your SSI application, it electronically transmits the determination to Washington's Automated Client Eligibility System, which opens Categorically Needy Apple Health coverage automatically.

No separate state application is required. No waiting period. The coverage begins the same month as SSI eligibility. This is the simplest and most common pathway for adults with developmental disabilities — apply for SSI, get approved, and Apple Health activates without any additional paperwork.

When SSI Isn't Approved (or Hasn't Been Yet)

If SSI is denied, delayed, or you don't qualify for financial reasons, Apple Health doesn't disappear as an option. Washington offers multiple non-SSI pathways:

ABD-linked Apple Health. If you qualify for the state's Aged, Blind, or Disabled cash assistance program, that eligibility also opens Apple Health. Since ABD is designed as a bridge during SSI processing, this pathway covers you while the federal application is pending.

MAGI Medicaid. Adults under 65 with income within the applicable MAGI limit can qualify for Apple Health through Modified Adjusted Gross Income rules, regardless of disability status. This is the standard Affordable Care Act Medicaid expansion pathway. Apply through Washington Healthplanfinder (wahealthplanfinder.org).

Non-MAGI Medicaid. For individuals whose financial situation doesn't fit MAGI criteria — typically because of resource-based limits rather than income — non-MAGI Apple Health uses the traditional Medicaid eligibility standards under WAC 182-512. Apply through the Washington Connection portal (washingtonconnection.org) or at your local DSHS Community Services Office using Form HCA 18-005.

Healthcare for Workers with Disabilities (HWD)

HWD is Washington's Medicaid buy-in for working individuals with disabilities. It's the most generous program on paper:

  • No income limit — earn any amount and remain eligible
  • No asset limit — save any amount without jeopardizing coverage
  • Minimal work requirement — any amount of documented, taxable employment qualifies
  • Sliding-scale premium — capped at 7.5% of monthly net income

HWD is particularly important for DDA waiver and Community First Choice clients who work. Standard Medicaid rules would require working waiver clients to spend "excess" earnings on the cost of their personal care. HWD exempts them from this post-eligibility treatment of income — they pay the calculated premium and keep the rest.

What Happens When SSI Stops

If SSI benefits are suspended or terminated due to employment earnings or DAC income, Apple Health doesn't automatically end. Under the Phillips v. Scalese consent decree and WAC 182-504-0125, the Health Care Authority must conduct an ex parte review of the beneficiary's records before terminating coverage.

This review must evaluate every alternative Medicaid program: Section 1619(b) for workers whose SSI cash hit zero, Section 1634(c) DAC protection, HWD, and standard MAGI eligibility. Coverage continues uninterrupted while the review is pending.

If you receive a notice that Apple Health is being terminated after a change in SSI status, don't accept it at face value. Request verification that the ex parte review was completed and that all alternative programs were evaluated.

Section 1619(b): Medicaid While Working

When a working SSI recipient earns enough that their cash benefit drops to zero, Section 1619(b) preserves Apple Health coverage. The individual must still meet the medical definition of disability, keep countable assets below $2,000, and earn below Washington's state-specific threshold — $73,897 per year in 2026.

Section 1619(b) provides free Categorically Needy coverage with no premium. For workers earning below the threshold, it's better than HWD because there's no premium. For workers earning above the threshold, HWD is another coverage pathway to examine, subject to its eligibility rules.
