West Virginia Adult Services After the IEP Ends: What Comes Next

The Service Cliff Is Real

The day your child exits the West Virginia public school system — whether by graduation or by aging out at 21 — every school-based support stops. The therapies, the transportation, the daily structure, the paraprofessional support, the related services written into the IEP — all of it ends.

Unlike the school system, adult services in West Virginia are eligibility-based, not entitlement-based. No single agency replaces the school. Instead, families must coordinate across multiple disconnected systems: the Division of Rehabilitation Services for employment, the Bureau for Medical Services for Medicaid waivers, the Social Security Administration for income, and community providers for day-to-day support.

The transition from a structured, legally guaranteed system to a fragmented, application-driven one is what professionals call the "service cliff." Planning for it should start years before exit, not at the final IEP meeting.

The Summary of Performance: Your Bridge Document

The Summary of Performance is a mandatory document that West Virginia schools must compile before a student with a disability exits special education. The SOP meeting must occur within a specific window — between 7 and 45 days before the student's graduation or final school exit date.

The SOP synthesizes:

• The student's most recent multidisciplinary evaluations
• Current academic and functional performance levels
• Accommodation and support needs that will continue into adulthood
• Recommendations for post-secondary accommodations

This document serves a practical purpose beyond compliance. It can support accommodation requests to adult service providers, community colleges, universities, and vocational programs under the ADA and Section 504. Some providers or schools may still require updated documentation or their own evaluation.

Ensure the SOP meeting happens on time and that both the special educator and the student collaboratively sign the document. Attach the most recent psychological evaluations — they are critical for Medicaid waiver applications and SSI disability determinations.

Division of Rehabilitation Services: Employment Support

DRS is the primary agency for employment services after the IEP ends. If your child was already receiving Pre-ETS or had an active Individualized Plan for Employment during high school, the transition should be relatively smooth — the DRS counselor already knows them.

If your child was not connected to DRS during school, a post-school referral is still possible. DRS provides:

• Vocational assessment and career counseling
• Job placement assistance
• On-the-job training and job coaching
• Assistive technology for the workplace
• College tuition assistance (in some cases)

As of mid-2026, all DRS priority categories are open, meaning there is no waitlist for VR services. This could change if state funding is reduced.

For individuals with significant disabilities who need ongoing job coaching beyond the DRS stabilization period, long-term supported employment is funded through the Medicaid IDD Waiver — which is why getting on the waiver waitlist early matters.

SSI and Medicaid at Age 18

When a student turns 18, the Social Security Administration stops counting parental income and evaluates only the student's own income and assets. For many students with disabilities, this makes age 18 the optimal time to apply for adult SSI benefits.

West Virginia is a 1634 state, which means SSI approval automatically triggers Medicaid enrollment. No separate Medicaid application is needed. This is important because Medicaid coverage is a prerequisite for IDD Waiver services, Personal Care services, and many community-based supports.

The $2,000 asset limit applies. Any countable assets above this threshold will disqualify the individual from SSI and Medicaid. This is where special needs trusts become essential — assets held in a properly structured third-party trust are not counted against the limit.

Medicaid Personal Care as a Safety Net

For adults who do not qualify for the IDD Waiver or who are stuck on the waitlist, Medicaid Personal Care is a critical backup. Unlike the waiver, Personal Care is an entitlement — if the individual meets the medical and financial criteria, there is no waitlist.

Eligibility requires active Medicaid coverage and a documented clinical need for hands-on assistance with at least three Activities of Daily Living (eating, bathing, grooming, toileting, transferring, or walking). A physician, physician assistant, or advanced practice registered nurse must complete and sign a Personal Care Medical Necessity Evaluation Request form, which goes to Acentra Health for review.

Under West Virginia's participant-directed "Personal Options" model, family members — other than legally responsible spouses or court-appointed guardians — can be hired and paid as caregivers.

Building the Post-School Plan Before Exit

The strongest approach is to begin connecting with adult agencies during the student's final two to three years in school, not after exit. The IEP's transition plan should document specific handoff timelines:

• DRS referral and IPE development — ideally by junior year
• IDD Waiver application — by age 16–17 to start the waitlist clock
• SSI application — at or shortly before age 18
• Community provider research — day programs, supported employment, independent living options
