Age 12–14: Lay the Foundation

• [ ] Level 1 Career Assessment completed — New York districts must conduct an initial vocational assessment, including student and parent interviews, by age 12. This assessment is updated annually and feeds into transition planning.
• [ ] Preserve all clinical records — Gather and organize psychological evaluations, adaptive behavior assessments (Vineland or ABAS scores), and developmental history documentation. These records will be needed for OPWDD eligibility, SSI redetermination, and guardianship proceedings.

Age 15: School Transition Begins

Age 17: The Evidence Assembly Year

• [ ] Compile SSI redetermination documentation — Assemble medical records, IEPs, teacher/employer reports, and functional descriptions that address the adult disability standard (inability to perform Substantial Gainful Activity). This documentation must be ready before the 18th birthday.
• [ ] Evaluate legal decision-making options — Research Supported Decision-Making Agreements (under MHL Article 82), representative payee designation, and guardianship (Article 17-A plenary or Article 81 tailored). Consult with a facilitator or attorney if guardianship may be needed.
• [ ] Complete OPWDD basic eligibility — Follow up with the DDRO to confirm that the eligibility determination is complete. Any delays here cascade into CCO enrollment and HCBS waiver access.

Age 18: The Legal and Financial Shift

• [ ] File adult SSI application — For individuals not previously on SSI, apply immediately after the 18th birthday. Parental income deeming ends at 18, so the young adult is evaluated on their own income and resources.
• [ ] Prepare for SSI age-18 redetermination — For current SSI recipients, the SSA will send a redetermination notice. Complete Form SSA-3368 (Disability Report for Adults) and Form SSA-827 (Authorization to Release Information) with comprehensive documentation.
• [ ] Confirm Medicaid activation — Monitor the automatic Section 1634 data transfer from SSA to the NYS Department of Health. If coverage doesn't appear within 60 days, file Form DOH-5104 or DOH-4220 (Access NY Health) with the local DSS to manually activate.

Age 18–21: Stabilize and Build

• [ ] Complete the Coordinated Assessment System (CAS) — Schedule the Maximus-administered functional assessment to establish support needs and move toward HCBS waiver authorization.
• [ ] Submit HCBS Waiver application — Work with your Care Manager to file the application once the CAS is complete and the LCED is approved.
• [ ] Secure actual service providers — Once OPWDD authorizes services, locate local providers with available staff capacity for day habilitation, community habilitation, supported employment, or self-direction.
• [ ] If SSI is denied, request Statutory Benefit Continuation within 10 days — This keeps cash payments and Medicaid active during the appeal. File Form SSA-561 (Request for Reconsideration) within 60 days.
• [ ] Explore DAC eligibility — If a parent is retired, receiving SSDI, or deceased, apply for Disabled Adult Child benefits at the local SSA field office. Ensure SSI and Medicaid are established first to preserve Section 1634(c) Medicaid protection.

Age 21–22: School Exit

• [ ] Maintain school enrollment until the day before the 22nd birthday — Under the A.R. v. Connecticut ruling and NYSED Opinion No. 242, New York districts must provide FAPE until the day before the student's 22nd birthday (unless they graduate with a regular diploma first).
• [ ] Complete the Student Exit Summary — Work with the CSE to finalize this document, which summarizes academic achievement, functional performance, and recommended accommodations.
• [ ] Confirm adult services are ready — Ensure OPWDD service authorizations are active and providers are engaged before school services end. The Care Manager should submit requests to the regional DDRO several months before the 22nd birthday.
• [ ] Transition ACCES-VR to full application — If the student was accessing Pre-ETS under a potentially eligible referral, file a formal ACCES-VR application to transition to adult vocational rehabilitation, job coaching, and customized employment planning.
