Oklahoma Disability Transition Checklist

The transition from childhood disability services to the adult system in Oklahoma involves four separate agencies (SSA, OKDHS, DRS, and the school district), each with their own application processes, eligibility criteria, and deadlines. Missing a single step can create months-long gaps in income, healthcare, or daily support services. This checklist maps the critical actions by age, so families can track progress against the timeline rather than discovering missed deadlines after the fact.

Age 15 / 9th Grade

Oklahoma mandates transition planning in the IEP earlier than the federal requirement — by the beginning of ninth grade or upon turning 15, whichever comes first.

  • [ ] Verify that formal secondary transition goals are written into the IEP and are in active effect
  • [ ] Ensure age-appropriate transition assessments have been conducted in education, employment, and independent living
  • [ ] Request a referral to DRS for Pre-Employment Transition Services (Pre-ETS) — students do not need to be formal DRS clients for Pre-ETS
  • [ ] Begin building the transition evidence folder: collect all IEP evaluations, psychological assessments, therapy records, and clinical diagnoses

Age 15½

  • [ ] Submit a formal Application for Service to the Oklahoma Department of Rehabilitation Services through the online self-referral portal (oklahoma.gov/okdrs/self-refer.html) — this converts the student from "potentially eligible" to an active VR client
  • [ ] Begin developing the Individualized Plan for Employment (IPE) with the assigned DRS counselor
  • [ ] Coordinate with the school to enroll in School Work Study (SWS) or Work Adjustment Training (WAT) if appropriate

Age 17

  • [ ] Confirm that the school district has provided the mandatory Transfer of Rights notice — must be given on or before the 17th birthday, with signed acknowledgments in the IEP
  • [ ] Decide on the legal authority pathway: Supported Decision-Making, powers of attorney, limited guardianship, or a combination
  • [ ] If pursuing SDM, begin drafting the custom Supported Decision-Making agreement (Oklahoma does not provide a standardized form)
  • [ ] If pursuing guardianship, retain an attorney or obtain the county-specific pro se petition packet from the District Court probate division
  • [ ] Initiate mandatory background checks for guardianship: OSBI criminal history, sex offender registry, and OKDHS CANIS search (up to $35 per adult household member)

6 Months Before 18

At 18 (Month of Birthday)

  • [ ] Apply for adult SoonerCare through OKDHS Live (okdhslive.org) using Form 08MP003E — do not wait for SSI approval
  • [ ] Execute a written room-and-board agreement if the young adult remains in the family home (prevents the one-third SSI reduction)
  • [ ] If the young adult was on TEFRA (Katie Beckett), verify that the adult SoonerCare application is filed before TEFRA coverage terminates
  • [ ] Finalize and execute the SDM agreement, healthcare POA, and/or financial POA
  • [ ] If guardianship was filed, attend the hearing and obtain the letters of guardianship

Ages 18-22 (Ongoing)

  • [ ] Maintain regular contact with the DDS case manager — attend Regional Family Meetings when the young adult's cohort is called
  • [ ] Monitor the SSI redetermination decision — if denied, file the appeal and benefit continuation request within 10 days of receiving the notice
  • [ ] If the young adult is in the extended transition program (alternate diploma pathway), coordinate school exit timing with DDS waiver activation
  • [ ] Explore DRS-funded employment programs: School Work Study, Work Adjustment Training, and Project SEARCH for the final year of school eligibility
  • [ ] Apply work incentives: SEIE ($2,410/month, $9,730/year for students under 22), PASS plans, and IRWE deductions
  • [ ] Do not graduate the student with a standard diploma until adult agency services are in place

At School Exit (Age 22 or Graduation)

  • [ ] Confirm that DDS waiver services and a provider (or self-directed services) are active before school services terminate
  • [ ] Transition from school-funded job coaching to DRS-funded employment support
  • [ ] Review and update the IEP Summary of Performance for use as ongoing documentation
  • [ ] Verify that SoonerCare enrollment, SSI payments, and any applicable DAC benefits are all active and correctly coordinated
