Iowa IEP Letter Templates: Meeting Requests, Disagreements, and Complaints

The most important communication skill in Iowa special education advocacy is putting things in writing. Verbal conversations in the hallway after pickup do not create legal records. Emails and letters do. When you submit a written request citing specific Iowa law, the district and AEA are on notice — and their response (or failure to respond) becomes part of a documented record that matters in every dispute that might follow.

These templates are written for Iowa's specific legal framework. They reference Iowa Administrative Code Chapter 41, not just federal IDEA law, because Iowa's unique AEA structure and procedural requirements mean that generic national templates leave gaps.

Template 1: Requesting an IEP Meeting

Use this when you need to call an IEP meeting outside of the annual review schedule — to address an urgent service concern, a placement change, insufficient progress, or a staffing disruption. Iowa law requires the district to schedule the meeting at a mutually agreed time and place.

I am writing to formally request an IEP team meeting for my [son/daughter], [Child's Name], a student at [School Name]. I am requesting this meeting pursuant to my rights under the Individuals with Disabilities Education Act (20 U.S.C. § 1414(d)(4)) and Iowa Administrative Code Chapter 41.

The reason for this request is: [describe the specific concern clearly — for example: "missed speech-language therapy sessions over the past six weeks due to an unfilled AEA position," or "my child's quarterly progress report shows no measurable progress toward the reading fluency goal," or "a proposed change to my child's placement has not been presented to me in writing"].

I am requesting that the following personnel attend this meeting: [list the team members whose presence you need — AEA speech-language pathologist, AEA occupational therapist, the LEA representative with authority to commit district resources, etc.].

Please provide me with at least three proposed dates and times within the next ten school days. I am available [list your general availability] and am committed to finding a time that works for all required team members.

If the district or AEA intends to decline this meeting request, please provide a written Prior Written Notice explaining the reason for denial, pursuant to Iowa Administrative Code 281-41.503.

Template 2: Documenting Disagreement with an IEP Decision

Use this after an IEP meeting where the team made a decision you disagree with — a service denial, a proposed placement, a refusal to evaluate — and you want to formally document your objection and request a Prior Written Notice.

I am writing to formally document my disagreement with a decision made during [Child's Name]'s IEP meeting held on [date]. I am submitting this disagreement in writing and requesting a Prior Written Notice pursuant to my rights under Iowa Administrative Code 281-41.503 and 20 U.S.C. § 1415(b)(3).

The decision I am disagreeing with: [Describe exactly what the team decided — for example: "The team refused to increase [Child's Name]'s speech-language therapy from 30 minutes per week to 60 minutes per week," or "The team proposed moving [Child's Name] from the general education classroom to a self-contained special education setting."]

The basis for my disagreement: [Describe your reasoning — for example: "The progress monitoring data from the past two quarters shows no measurable gains under the current service frequency. I believe the intensity of services is insufficient to enable [Child's Name] to make progress toward the IEP goals." Or: "I do not believe the data supports a more restrictive placement. I am requesting documentation of what supplementary aids and services were attempted in the general education setting and why they were insufficient."]

I am requesting that the district/AEA provide me with a written Prior Written Notice that includes: (1) the action the agency is proposing or refusing; (2) an explanation of why the agency is proposing or refusing the action; (3) a description of each evaluation procedure, assessment, record, or report the agency used as a basis for the decision; and (4) a description of any other options the team considered and the reasons those options were rejected.

This Prior Written Notice must be provided in writing, in a form understandable to me, pursuant to Iowa Administrative Code 281-41.503. I expect to receive it within a reasonable time following this request.

Template 3: Requesting a Special Education Evaluation

Use this to formally request a full and individual evaluation for a child not currently in special education. The district and AEA have 60 calendar days from receiving your signed consent to complete the evaluation.

I am requesting this evaluation because I suspect [Child's Name] has a disability that adversely affects [his/her/their] educational performance and may require specially designed instruction. [Briefly describe the specific concerns you are observing — for example: "difficulty decoding printed text despite two years of reading intervention," or "significant behavioral challenges in the classroom that impair [his/her] ability to access instruction and maintain appropriate relationships with peers and adults."]

I understand that under Iowa Administrative Code 281-41.301, the district and AEA have 60 calendar days from the date of my signed parental consent to complete the evaluation and hold an eligibility determination meeting.

I further understand that implementing MTSS or RTI interventions cannot be used to delay or deny this evaluation request under Iowa Administrative Code 281-41.226(3).

Please contact me within five business days to discuss the evaluation plan and to provide me with the consent form. If you are declining to evaluate, please provide a Prior Written Notice explaining the basis for that refusal, as required by Iowa Administrative Code 281-41.503.

Template 4: Notice of Intent to File a State Complaint

Use this to notify the district and AEA that you are preparing to file a formal state complaint with the Iowa Department of Education. Sometimes this notice alone prompts the district to resolve the issue. Whether or not it does, filing the complaint is your legal right.

The specific violation: [Describe the specific failure clearly — for example: "The IEP dated [date] requires 60 minutes per week of speech-language therapy delivered by an AEA speech-language pathologist. According to ACHIEVE service logs and my own documentation, [Child's Name] received fewer than 20 minutes of speech service during the period from [date] to [date], a gap of more than 40 minutes per week."]

The data supporting this claim: [Briefly describe your documentation — ACHIEVE portal logs, your own session log, written communications from the school, etc.]

The Iowa DOE's state complaint process has a 60-calendar-day resolution timeline. I am providing this notice prior to filing so that the district and AEA have an opportunity to resolve the matter. If I do not receive a written response within ten school days addressing the violation and providing a corrective action plan, I will proceed with filing the state complaint.

Using These Templates Effectively

Send letters by email when possible — the timestamp creates an automatic record. Follow any email with a brief note to the recipient: "Please confirm receipt of this message." Keep copies of everything you send and receive in a dedicated folder.

When you receive a Prior Written Notice, read it carefully against the legal requirements: the agency must explain what they are proposing or refusing, why, and what data they used. A PWN that is vague or that references data you have never seen is itself a compliance issue worth challenging.
