How to Apply for Rhode Island DD Services Through BHDDH

Start at 16 Years and 10 Months

BHDDH's Division of Developmental Disabilities operates under an "eligibility by 17" policy, which means they want families to submit applications when the student is 16 years and 10 months old. That's not a hard legal deadline — it's a practical one. Starting early gives BHDDH time to verify clinical eligibility before the student exits school, so adult funding can be aligned with the school-year exit date.

Families who wait until 18 or later often face a gap between losing school services and receiving adult DD services. The early timeline is designed to prevent exactly that.

Clinical Eligibility Criteria

To qualify for BHDDH adult DD services, the applicant must meet the clinical definition under Rhode Island State Law § 40.1-21-4.3:

  • A documented intellectual or severe chronic physical impairment that manifested before age 22 and is likely to continue indefinitely
  • Substantial functional limitations in at least three of seven major life activities: self-care, receptive/expressive language, learning, mobility, self-direction, capacity for independent living, or economic self-sufficiency
  • For intellectual disability specifically: an IQ score below 70 alongside significant deficits in adaptive behavior

The clinical determination is processed by BHDDH's Division of Developmental Disabilities at Simpson Hall, 6 Harrington Road, Cranston, RI 02920. The main intake phone is (401) 462-3421, with a Spanish intake line at (401) 462-3014.

Required Documentation

The application package needs to include:

  • Psychological evaluations with cognitive test scores (Wechsler, Stanford-Binet, or equivalent)
  • Clinical DSM diagnosis — Autism Spectrum Disorder, Intellectual Disability, Down Syndrome, or related developmental conditions
  • Medical history documenting the onset before age 22
  • IEP records including the most recent transition plan
  • School vocational records and Career Development Plans
  • State Assigned Student Identifier (SASID) number

BHDDH issues a clinical eligibility determination within approximately 30 days of receiving a complete application. Incomplete packages get sent back, so double-check that every required document is included before mailing.

The Dual-Agency Application Process

BHDDH eligibility is only half the picture. Because all adult DD services are funded through Medicaid, the young adult must also be determined eligible for the Long-Term Services and Supports (LTSS) waiver through the Department of Human Services.

The sequence works like this:

  1. Submit the BHDDH clinical application (at age 16 years, 10 months)
  2. Submit the DHS-2 Medicaid application (at age 18, when the young adult is evaluated as a "family of one")
  3. Complete the SIS-A assessment (scheduled by BHDDH after clinical eligibility is confirmed)
  4. Select a service delivery model and begin provider matching

For the DHS-2 Medicaid application, the financial eligibility standards for LTSS are a $4,000 single resource limit and a monthly income ceiling of $2,982 (300% of the 2026 federal benefit rate).

What Happens After Eligibility

Once BHDDH confirms clinical eligibility, they schedule a Supports Intensity Scale (SIS-A) assessment — a two-hour evaluation that measures the pattern and intensity of support the individual needs. The SIS-A score determines the funding tier (A through E), which sets the annual service budget.

From there, the family selects either an agency-managed or self-directed service delivery model, and a Conflict-Free Case Management (CFCM) agency develops the Person-Centered Plan within 45 calendar days of initial contact.
